The mental health area includes a rate of medical follow-up in the community after discharge from psychiatric hospitalization. Improving the continuum of treatment contributes to reducing complications and improving responsiveness to continued mental health care. In addition, a number of measures are included to monitor physical morbidity in people with severe mental illness (schizophrenia or bipolar disorder). Cardiovascular disease is more common in people with severe mental illness than in the general population, hence the importance of monitoring. The measures: the rate of diabetes prevelence, hemoglobin A1c testing, and diabetes control, as well as monitoring body weight and the rate of obesity in people with severe mental illness.
